Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in New Haven, CT. Available positions include Full Time, Part Time, or PRN.

The OTA will plan, organize, develop, and direct occupational therapy services, screen residents, implement treatment plans, document and bill, and collaborate with a multidisciplinary team to ensure high-quality resident care.

Qualifications

• Must hold active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment per scope of practice.
• Perform treatment plans as directed by supervising therapist and established goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and interdisciplinary team members.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing of services.
• Perform all other duties as required to effectively treat patients.
• Comply with CMS documentation standards in daily practice.
