Evolve Therapy Services seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in New Haven, CT. This full-time/part-time/PRN position requires active OTA licensure and an associate’s degree in Occupational Therapy.

You will screen residents, implement treatment plans, document care, and collaborate with an interdisciplinary team to ensure high-quality resident care consistent with CMS standards.

Formación

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsabilidades

• Screen residents to determine best intervention per scope of practice.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Conduct therapy according to the treatment plan and document/bill services.
• Comply with CMS documentation standards daily with point of care documentation.
• Perform all other duties as assigned to effectively treat patients.
