Trio Health Partners seeks an In-House Nurse Practitioner to act as the primary internal medicine provider within a Skilled Nursing Facility. You will build strong relationships with residents, families, staff, and other providers while contributing to the clinical leadership team.

We focus on proactive, preventive care for geriatric residents in skilled nursing and long-term care settings. The role offers flexible full-time schedules with no nights or weekends and a competitive salary plus

Qualifications

• Active Illinois Nurse Practitioner license (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with skilled nursing/long-term care focus.
• Ability to engage and communicate effectively with elderly patients and families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing target.
• Knowledge of Medicare, Medicaid, and Health Plan benefits is a plus.

Responsabilités

• Provide essential care and leadership within the facility as In-House NP.
• Conduct new admissions evaluations and monthly visits for long-term residents.
• Perform comprehensive exams and evaluate condition changes.
• Prescribe medications and treatments as needed.
• Develop patient care policies and facilitate referrals to specialists.
• Coordinate advanced care planning and discharge visits.
• Maintain timely communication and documentation of assessments and plans.
