Job summary

Flagship Therapy is seeking a licensed Occupational Therapist for our Mt. Angel, OR in-house rehab program. This full-time role focuses on comprehensive evaluation, treatment planning, and patient-centered care for post-acute and geriatric populations.

You will screen and treat cognitive and ADL impairments, document progress, and collaborate with a multidisciplinary team to support recovery and discharge planning. Competitive benefits and a supportive work culture are offered.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL functioning disorders and develop plan of care following standards.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled treatment for cognitive and/or ADL disorders per clinical requirements.
• Administer procedures as part of the rehab plan including ADL retraining, exercises, modalities, and assistive devices.
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate outcomes and adjust services; refer as needed.
• Provide counseling to patients and families about physical disorders.
• Communicate progress and plans with supervisor and health team.
• Prepare comprehensive discharge summary per regulatory requirements.
