Job summary

Wellstone in Cullman, AL is seeking an experienced In-Home Therapist to assess crisis needs, provide direct youth and family services, and monitor client progress in community settings. The role emphasizes psychosocial interventions, crisis management, and coordination with co-therapists and medical staff.

Monday–Friday, 8:00 AM–5:00 PM, with up to 80% travel and office/community-based duties. The ideal candidate holds a Master's degree in a related field, 2+ years post-master's experience with

Qualifications

• Master's degree in a related field (education level)
• Two years post-master's experience with SMI population preferred
• Excellent verbal, written communication & planning skills
• Knowledge of community agencies, Medicaid, & Department of Human Resources
• Excellent 5-year motor vehicle driving record; Reliable transportation

Responsibilities

• Work in frequent contact with co-therapist and medical personnel to provide mental health treatment in the community (schools, home, etc).
• Conduct intake assessments, CANS, and make referrals from assessments.
• Provide individual, family, and group therapy for clients three times per week for a minimum of 12 weeks in the community.
• Provide 24 hours availability to clients in the program and community organizations.
• Assist clients in establishing and maintaining a social support network within the community.
• Complete paperwork in a timely manner per Center/DMH/MR standards.
• Visit clients in their environment to assess progress towards goals and provide support.
• Advocate for clients and explain diagnoses, treatment approaches to families.
• Monitor medications and relay information to co-therapist.
• Perform crisis intervention as needed and/or required.
• Work with Probation Offices to provide weekly reports on clients.
• Participate in trainings and mandated meetings.
• Complete other duties as assigned.
