You will design personalized plans and work with a collaborative care team to empower youth to thrive in homes, schools, and communities. Qualifications

• Must be 21+ years old.
• Background check and child abuse registry clearance per MT/state requirements.
• Bachelor's degree in a human services field or equivalent experience (6 years experience = bachelor’s).
• Experience and ability to work with youth and families in diverse settings.

Responsibilities

• Empower Families: assess needs and deliver targeted interventions.
• Design individualized strategies using evidence-based practices.
• Deliver care in homes, schools, and community settings.
• Collaborate with treatment teams, educators, and families on comprehensive plans.
• Complete required documentation in EMR per agency standards.
• Support PQI initiatives and contribute to agency goals.
• Demonstrate professionalism and uphold YBGR values.
