Qualifications

• Must have a bachelor’s degree or equivalent in a human services field.

Responsibilities

• Empower Families by assessing needs and delivering targeted interventions.
• Design individualized plans using evidence-based practices to support youth.
• Deliver care in homes, schools, and communities in line with agency standards.
• Collaborate with treatment teams, educators, clinicians, families, and community stakeholders to build comprehensive treatment plans.
• Accurately complete all required documentation in the EMR system per agency standards.
• Contribute to agency-wide goals through participation in PQI initiatives.
• Represent YBGR with professionalism and uphold core values.
• Uphold safety and compliance with policies and procedures.
• Identify challenges and proactively seek solutions benefiting youth and families.
• Engage in regular supervision, coaching, and ongoing training.
• Serve with cultural humility and awareness of socioeconomic backgrounds.
• Take on additional responsibilities as needed to support the mission.
