Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide in-home health evaluations across a defined coverage area. The primary work location is the patient’s home, with up to 100% travel; overnight stays may be required depending on territory.

The role involves evaluating medical history, performing physical exams, and delivering diagnostic services as ordered by health plans. Virtual visits may be conducted as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsabilités

• Conduct in-home health evaluations in patients' homes.
• Perform medication and medical history reviews and physical evaluations.
• Conduct virtual visits as needed.
• Travel locally within licensed states.
