Job summary

Capital Health in New Jersey is seeking a dedicated Home Health Registered Nurse to provide patient care in home settings and via our command center. The role emphasizes care coordination, telehealth assessments, and collaboration with a multidisciplinary team to achieve optimal patient outcomes.

You will document care in the EMR, adhere to nursing standards, and support discharge planning while pursuing ongoing professional development.

Qualifications

• Education: Associate's degree or higher in nursing; BSN preferred.
• Experience: Three years in an acute care setting; telemetry experience required.
• Licenses/Certs: NJ RN or multi-state RN; ACLS and BLS required (or must obtain within 6 months); NIH Stroke Scale certification within orientation.

Responsibilities

• Deliver in-home nursing care to patients enrolled in the Hospital at Home program.
• Conduct virtual nursing assessments via video from the on-site command center.
• Document in EMR per NJ Board of Nursing and CMS standards.
• Triages patient needs and escalates as needed; ensure patient safety per policies.
• Develop discharge plans and educate patient and family for ongoing care.
