Job summary

Housecall Providers is seeking a Physician Assistant to deliver primary care in patients' homes and via telehealth, focusing on chronic disease management and palliative support. You will collaborate with physicians to diagnose, treat, and follow-up for complex patients in Salem and surrounding regions.

Responsibilities include conducting health assessments, formulating care plans, coordinating referrals, and educating patients and families on preventive care and medication use.

Qualifications

• Master’s level education with major course work in primary care for physician assistants or related field.
• Licensed physician assistant in the State of Oregon.
• Drug Enforcement Administration (DEA) license.
• Certification by NCCPA.
• CPR certification at hire or within 6 months in position.

Responsibilities

• Provide diagnosis, treatment, consultation, and follow-up care under collaboration with a licensed physician.
• Perform comprehensive health assessments and identify health needs.
• Address prevention, chronic disease management, and end-of-life care goals with patients and families.
• Develop and implement proactive patient-centered care plans with the care team.
• Order testing and medications with follow-up and reassessment; provide continuity of care.
