Job summary

Housecall Providers Inc. seeks a Physician Assistant / PA to deliver primary care in Salem, OR, focusing on chronic disease management and palliative care in patients' homes and via audio/video encounters.

The role covers regular care with long-term disease prevention and requires Oregon PA licensure, NCCPA certification, and a DEA license; includes after-hours call and a comprehensive benefits package.

Qualifications

• Master’s degree in a primary care PA program or related field.
• Licensed physician assistant in Oregon with NCCPA certification.
• DEA license required for prescribing controlled substances.
• CPR certification at hire or within 6 months.
• Valid driver’s license and acceptable driving record.

Responsibilities

• Provide diagnosis, treatment, consultation and follow-up care with physician collaboration.
• Perform comprehensive health assessments including physical and psychosocial history.
• Make referrals to social or medical services as needed.
• Develop proactive patient-centered care plans with the care team.
• Order tests and medications; manage referrals and aftercare.
• Deliver high-quality palliative care aligned with patient goals.
