Community Options, Inc. in the Greater Phoenix area is hiring both full-time and part-time In Home Support Providers (DSP) to assist individuals with learning daily living skills and community integration.

As a DSP, you will support individuals to increase independence, meet them in various settings, document progress, and coordinate activities while ensuring safety and proper medication administration. A high school diploma or GED and a valid driver's license are required, with English

• Effective communication in English with participants, staff, and supervisors.

Mansioni

• Support individuals to increase independence.
• Meet waiver participants regularly in residence, community, or office.
• Provide ongoing support for community integration.
• Document progress toward community integration.
• Coordinate leisure and volunteer opportunities.
• Administer medications and ensure proper documentation.
• Drive to medical appointments and activities.
• Complete program documents and billable records accurately.
• Cooperate with staff during inspections.
