Job summary

Hacienda HealthCare in Phoenix seeks Direct Support Professionals (DSPs) to provide person-centered care across group homes, day treatment for adults, and employment programs. You will assist with daily living, mobility, health monitoring, and community integration under supervision.

This safety-sensitive role requires flexible hours, driving duties when assigned, and meticulous documentation to support ISP goals and resident well-being.

Qualifications

• Must be at least 25 years old to drive for the organization.
• Must have a clean driving record and be approved by our insurance carrier.
• Must complete Hacienda Drivers Training.
• Flexible hours and locations to assist with staff coverage.
• Reliable transportation to work at any location.
• Must obtain and maintain CPR/1st Aid within the specified timeframe.
• Must have or be capable of passing a drug screen.
• Must have or be capable of obtaining a Class 1 Finger Print Clearance Card.

Responsibilities

• Provide safety, emotional, and physical support to residents.
• Assist with activities of daily living and community integration.
• Position residents every hour awake and as needed while asleep.
• Document ISP objectives and data daily.
• Assist residents with meals and hydration.
• Ensure residents are clean, well-groomed and appropriately dressed.
• Drive residents to appointments and activities as scheduled.
• Maintain a clean driving record and report changes.
• Complete required documentation before end of shift.
