Job summary

Blue River Services, Inc. is seeking a Direct Support Professional to provide in-home and community-based support to individuals with disabilities.

This role emphasizes promoting independence, life skills development, and helping clients achieve goals while remaining in their home environment. Responsibilities include assessing needs, creating service plans, training in daily living skills, and assisting with community access.

Qualifications

• Strong interpersonal and communication skills.
• Ability to work independently in home and community settings.

• Ability to collaborate effectively with team members and families.
• Basic computer skills (word processing, spreadsheets, and data entry).
• Experience with individuals with disabilities is required (noted in Education).

Responsibilities

• Assess individual needs and develop and implement individualized service plans.
• Provide training in daily living skills, including personal care, communication, and social development.
• Support individuals in accessing community resources and participating in community activities.
• Assist with household tasks, organization, and maintaining a safe home environment.
• Escort individuals to medical appointments and community activities as needed.
• Support individuals with correspondence, budgeting, and bill paying when necessary.
• Maintain accurate documentation of services, progress, and outcomes in compliance with regulations.
• Develop materials and maintain supplies to support individual goals.
• Act as a resource to families, caregivers, and other team members.
• Promote independence, self-advocacy, and personal growth.
• Maintain positive relationships with individuals, families, and community partners.
• Remain flexible to meet the needs of individuals served.
