Job summary

HopeWell, Inc. is seeking an In-Home Support Worker to assist adults with intellectual and developmental disabilities in Roxbury, MA. The role focuses on promoting independence, skill-building, and community integration within individuals’ homes and communities.

The position requires empathy, strong communication, and organizational skills to develop individualized plans and coordinate services with families and external partners. Some driving and home-based work are expected.

Qualifications

• BA in human services field or High School Diploma/associate degree plus 2 years relevant experience. MA degree in human service field a plus.
• Ability to develop authentic partnerships and advocate for individuals with different abilities.
• Willingness to work in home and community settings with occasional driving.

Responsibilities

• Collaborate with supervisor, Family Support Specialist, and individuals to set goals in daily living, career needs, social and recreational activities.
• Develop and implement the Individuals’ Service Plan (ISP) reflecting the individual’s preferences, goals, lifestyles, and interests.
• Build positive relationships with individuals and their children in home or community to provide direct support and coaching.
• Schedule and attend appointments and services and provide transportation when needed.
• Create teaching moments to develop skills in personal finance, health, shopping, and community resources.
