Job summary

HopeWell, Inc. in Roxbury, MA, is seeking a Youth Advocate to support transition-age youth through community-based services, housing stability, and education/employment support.

The role involves direct youth engagement, case management, and collaboration with a multidisciplinary team to ensure comprehensive care. The ideal candidate has a master’s degree in social work or related field and experience with trauma-informed, youth-centered approaches.

Qualifications

• Master’s degree in social work, mental health counseling, or related field preferred.
• Experience working with transition-age youth with complex presentations, including chronic trauma, preferred.
• Must be comfortable working in community settings.
• High level of personal accountability for the quality and impact of work.

Responsibilities

• Develop a partnership strategy with each youth, provide direct support, coaching, mentoring, and intervention to increase self-sufficiency and goal attainment.
• Provide youth with individual counseling, crisis intervention, referrals, advocacy, and life skills training; crisis counseling as needed.
• Assess youth well-being and develop plans to ensure safety and de-escalate crisis as needed.
• Provide clinical leadership to multi-disciplinary teams to ensure seamless service delivery and information sharing.
• Collaborate with community providers including social workers, courts, schools, and families.
• Complete intake paperwork, consents, Care and Services plans, needs assessments, and progress notes.
