Qualifications

• Must be at least 18 years old or 21 depending on the person-served.
• Education, training, or two years of equivalent experience in working with developmentally disabled and/or mental illness.
• Valid driver's license with acceptable driving record and reliable vehicle required.

Responsibilities

• Assist individuals in a 1:1 in-home and in-community setting.
• Provide skill development for community participation and health.
• Attend team meetings and write progress reports.
