Job summary

Ascend Services, Inc. is seeking compassionate HCBS Direct Support Professionals (DSPs) to provide personalized support to children and adolescents in their homes and communities in Brooklyn, NY.

You will assist with daily living activities, support personal goals, promote independence, and collaborate with families and healthcare professionals to implement personalized plans. A valid driver's license and reliable vehicle are required.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree preferred or equivalent.
• Valid driver's license and access to a vehicle.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work evenings and weekends.

Responsibilities

• Provide one-on-one support and assistance with daily living activities, such as personal care, meal preparation, and household chores.
• Assist individuals in developing and achieving their personal goals and outcomes.
• Encourage and promote independence, self-advocacy, and self-determination.
• Collaborate with individuals, families, and healthcare professionals to develop and implement personalized support plans.
• Maintain accurate records and documentation of services provided.
• Provide crisis intervention and support as needed.
