Job summary

Ascend Services, Inc. is seeking compassionate HCBS Direct Support Professionals (DSPs) to provide personalized support to children and adolescents in their homes and communities in Pawling, NY.

You will assist with daily living activities, help individuals pursue personal goals, promote independence, and collaborate with families and healthcare professionals to implement tailored support plans. This role emphasizes empowerment, community integration, and crisis support when needed.

Qualifications

• Excellent communication, interpersonal, and problem-solving skills required.
• Ability to work flexible hours, including evenings and weekends.
• High school diploma or equivalent required.
• Bachelor degree preferred or required depending on role.
• Valid driver's license and vehicle required.

Responsibilities

• Provide one-on-one support and assistance with daily living activities, such as personal care, meal preparation, and household chores.
• Assist individuals in developing and achieving their personal goals and outcomes
• Encourage and promote independence, self-advocacy, and self-determination
• Collaborate with individuals, families, and healthcare professionals to develop and implement personalized support plans
• Maintain accurate records and documentation of services provided.
• Provide crisis intervention and support as needed.
