Ascend Services, Inc. is seeking compassionate HCBS Direct Support Professionals (DSPs) to provide personalized support to children and adolescents in their homes and communities in Brooklyn, NY.

You will assist with daily living activities, support personal goals, promote independence, and collaborate with families and healthcare professionals to implement personalized plans. A valid driver's license and reliable vehicle are required.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree preferred or equivalent.
• Valid driver's license and access to a vehicle.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work evenings and weekends.

• Encourage and promote independence, self-advocacy, and self-determination.

• Maintain accurate records and documentation of services provided.
• Provide crisis intervention and support as needed.

DSP:

We are seeking compassionate and dedicated HCBS Direct Support Professionals (DSPs) to provide personalized support and care to children and adolescents in their homes and communities. As an HCBS DSP, you will play a vital role in empowering individuals to lead fulfilling lives, promoting independence, and fostering community integration.

Locations: Brooklyn, NY

Requirements:
