Job summary

Wellspring Lutheran Services in Clare, Michigan, seeks a Substance Use Disorder Family Specialist to deliver in-home assessment, treatment, and case management for families involved with Child Protective Services/MDHHS.

You'll conduct ASAM-based assessments, develop recovery-focused plans, provide in-home treatment over six months, and coordinate with MDHHS and community partners. Travel within Midland, Clare, Gladwin and Isabella counties is required.

Qualifications

• Master’s degree (or BA with strong experience) in a related field.
• Experience with individuals and families affected by substance use.
• MCBAP certification or CADC/CAADC development plan within 30 days of hire.
• Valid driver's license, vehicle registration, and auto insurance.
• Comfortable working in clients' homes.

Responsibilities

• Conduct comprehensive substance use assessments using ASAM criteria.
• Develop individualized treatment plans focused on recovery and family preservation.
• Provide in-home substance use treatment services over a six-month period.
• Deliver psychoeducation on substance use and parenting impacts.
• Facilitate relapse prevention planning and coping skills.
• Provide ongoing case management addressing housing and employment barriers.
• Collaborate with MDHHS and community partners; maintain documentation.
• Utilize safety planning to reduce risks for families.
• Attend weekly meetings and required trainings with partners and MDHHS.
• Support clients’ recovery pathways and transport as needed.
