Home Health of Western Colorado in Montrose, CO is seeking a Speech Therapist to provide in-home therapy under physician orders. This PRN position offers a meaningful chance to serve older adults and individuals with neurological disorders while joining a supportive, mission-driven team.

Applicants should have a master’s in Speech-Language Pathology (CCC preferred) or equivalent experience, a valid state license or license supervision plan, and be prepared to collaborate with families and health

Qualifications

• Master’s degree in Speech-Language Pathology (CCC preferred) or equivalent experience with adults/children with neurological disorders.
• Valid state license or actively accumulating supervisory experience required for license.
• ASHA membership recommended.

Responsibilities

• Evaluate and treat clients per physician orders; communicate progress to families and health team.
• Initiate treatments and monitor client progress with referring physicians.
• Maintain proper documentation per state, federal and third-party payor regulations.
• Comply with the Speech Language Pathology Practice Act in state.
• Perform other duties or special assignments as directed by the Director of Clinical Services.
