Job summary

Senior Helpers of Melbourne is seeking a compassionate Speech-Language Pathologist to join our home health clinical team on a PRN/contract basis. The SLP will provide skilled services in patients' homes, evaluating and treating communication, cognitive, speech, language, voice, and swallowing disorders in coordination with the care team.

Requirements include a current Florida license to practice SLP, a graduate degree in SLP, CPR/BLS, and the ability to travel to patients' homes throughout the

Qualifications

• Current and unrestricted Florida license to practice Speech-Language Pathology.
• Graduate degree in Speech-Language Pathology from an accredited program.
• CCC-SLP certification is preferred.
• Current CPR/BLS certification.
• Valid driver's license and ability to travel to patients' homes.

Responsibilities

• Perform comprehensive speech-language pathology evaluations in the patient's home.
• Evaluate speech, language, communication, cognitive, voice, and swallowing function as appropriate.
• Develop individualized treatment plans based on patient needs and goals.
• Provide skilled therapy for speech, language, cognitive-communication, and swallowing disorders.
• Provide dysphagia treatment and swallowing education when indicated.
• Educate patients and caregivers regarding treatment strategies and safety measures.
• Monitor patient progress and modify treatment plans as needed.
• Communicate changes in condition with the physician and care team.
• Complete required clinical documentation accurately and timely.
• Participate in care coordination and interdisciplinary communication.
• Follow all agency policies and state/federal requirements.
