Job summary

VitalCaring Group is seeking a dedicated Home Health Speech-Language Pathologist to evaluate and treat patients in home settings, focusing on communication, cognition, and swallowing.

You will work autonomously with clinical leadership support, travel between patient homes, and document care in the EMR to ensure coordinated, patient-centered outcomes.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology.
• Current SLP license in the state of practice.
• Minimum of 1 year clinical experience (home health preferred).
• CCC-SLP preferred.
• Strong clinical judgment and ability to work independently.
• Excellent communication and patient education skills.
• CPR certification required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead evaluation and treatment of speech, language, cognitive, and swallowing disorders in home health.
• Develop and implement individualized plans of care to improve communication and safety.
• Provide therapies using evidence-based techniques and tools.
• Assess home environments and recommend adaptive equipment or aids.
• Educate patients and caregivers on exercises and safety techniques.
• Document care accurately in the EMR and coordinate with the care team.
