Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to provide one-on-one in-home care in Ann Arbor, MI. You will evaluate, treat, and develop plans of care for older adults, helping them improve mobility and independence.

The role offers flexibility, the potential for telehealth integration, and opportunities to build strong patient relationships within home and senior living settings.

Qualifications

• Valid OT license in the state of practice or eligibility to obtain it.
• Degree from an accredited OT program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one OT services in clients' homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that improve mobility, independence and functional outcomes.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
