Job summary

FOX Rehabilitation in Valley Cottage, NY is seeking a licensed Speech Language Pathologist to deliver one-on-one geriatric care in private residences and senior living communities. You will evaluate, treat, and document using FOX’s EMR, while collaborating with an interdisciplinary team to promote independence and aging in place.

The role offers autonomy, flexible scheduling, and telehealth options to support patient access and continuity of care.

Qualifications

• Degree from an accredited Speech Language Pathology program.
• Valid Speech Language Pathology license in the state(s) of practice.
• ASHA CCC certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one speech-language pathology services to older adults in private homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve wellness, mobility, and functional independence.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
