Job summary

Palms Home Care is seeking a Speech Language Pathologist to provide in-home therapy services across Florida. The role requires independence while coordinating with an interdisciplinary team and ensuring Medicare-compliant documentation.

Qualifications include a Master’s in Speech Language Pathology and an active Florida SLP license, with home health experience preferred and CPR certification. A valid driver's license and reliable transportation are required.

Qualifications

• Master’s degree in Speech Language Pathology.
• Active Florida SLP license.
• Home health experience preferred.
• Knowledge of OASIS documentation is a plus.
• Possesses and maintains current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive evaluations and OASIS assessments
• Develop and implement individualized care plans
• Treat patients with speech, language, cognitive, and dysphagia disorders
• Accurately document visits in accordance with Medicare guidelines
• Educate patients and caregivers on exercises and compensatory strategies
• Communicate effectively with physicians and care team members
