Job summary

Palmshomecare in Port Charlotte, FL is seeking a dedicated Speech Language Pathologist to provide in-home therapy and coordinate care with an interdisciplinary team. The ideal candidate has home health experience, a valid Florida SLP license, CPR certification, and strong documentation skills in Medicare guidelines.

Qualifications

• Master’s degree in Speech Language Pathology.
• Active Florida SLP license.
• Home health experience preferred.
• Knowledge of OASIS documentation is a plus.
• Possesses and maintains CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive evaluations and OASIS assessments.
• Develop and implement individualized care plans.
• Treat patients with speech, language, cognitive, and dysphagia disorders.
• Accurately document visits in accordance with Medicare guidelines.
• Educate patients and caregivers on exercises and compensatory strategies.
• Communicate effectively with physicians and care team members.
