Job summary

Apollo Home Healthcare is seeking a Speech-Language Pathologist (SLP) to provide in-home speech therapy visits across Contra Costa County, Alameda County, and the Tri-Valley area. This W-2 per diem role offers schedule flexibility and autonomy while collaborating with a supportive interdisciplinary team.

You'll perform evaluations, develop plans of care, and deliver high-quality one-on-one care to patients in their homes, advancing communication and swallowing skills.

Qualifications

• Active California SLP license.
• Master's degree in Speech-Language Pathology.
• Current CPR certification.
• Home health experience preferred but not required.
• Reliable transportation and valid driver’s license.
• Strong documentation and communication skills.

Responsibilities

• Perform speech-language and swallowing evaluations in patients' homes.
• Develop and implement individualized plans of care.
• Provide skilled speech therapy interventions.
• Recommend communication devices and adaptive equipment as needed.
• Assess home environments for safety and functional communication needs.
• Collaborate with physicians, nurses, PT/OT, and care teams.
• Participate in case conferences as appropriate.
• Complete timely, compliant clinical documentation.
• Educate patients and caregivers on exercises and compensatory strategies.
• Supervise home health aides when applicable to the plan of care.
