Job summary

Apollo Home Healthcare is seeking a Speech-Language Pathologist (SLP) to provide in-home speech therapy visits across Contra Costa, Alameda, and the Tri-Valley area. This W-2 per diem role offers schedule flexibility, autonomy, and meaningful patient relationships, with a collaborative interdisciplinary team.

The Home Health SLP will evaluate, plan, and implement therapy, educate patients and caregivers, and document care clearly. A CA SLP license, Master's in SLP, and CPR are required.

Qualifications

• Active California SLP license required.
• Master's degree in Speech-Language Pathology required.
• Current CPR certification required.
• Home health experience preferred but not required.
• Strong documentation and communication skills required.
• Ability to work independently in the field.
• Reliable transportation and valid driver's license required.

Responsibilities

• Perform in-home speech-language and swallowing evaluations.
• Develop and implement individualized Plans of Care.
• Provide skilled speech therapy interventions.
• Recommend communication devices and adaptive equipment as needed.
• Assess home environments for safety and functional communication needs.
• Collaborate with physicians, nurses, PT/OT, and care teams.
• Participate in case conferences as appropriate.
• Complete timely, compliant clinical documentation.
• Educate patients and caregivers on exercises and compensatory strategies.
• Supervise home health aides when applicable to the plan of care.

What You'll Do
