Apollo Home Healthcare is seeking a Speech-Language Pathologist (SLP) to perform in-home visits across Contra Costa County, Alameda County, and the Tri-Valley area. This W-2 per diem role offers schedule flexibility, autonomy, and meaningful patient relationships.

As part of the Obran Cooperative, we provide a worker-owned model and supportive, interdisciplinary teamwork. You will assess, treat, and educate patients in their homes while maintaining high standards of care.

Qualifications

• Active California Speech-Language Pathologist license.
• Master's degree in Speech-Language Pathology.
• Current CPR certification.
• Home health experience preferred.
• Strong documentation and communication skills.
• Ability to work independently in the field.
• Reliable transportation and valid driver's license.

Responsibilities

• Perform speech-language and swallowing evaluations in patients' homes
• Develop and implement individualized Plans of Care
• Provide skilled speech therapy interventions
• Recommend communication devices and adaptive equipment as needed
• Assess home environments for safety and functional communication needs
• Collaborate with physicians, nurses, PT/OT, and care teams
• Participate in case conferences as appropriate
• Complete timely, compliant clinical documentation
• Educate patients and caregivers on exercises and compensatory strategies
• Supervise home health aides when applicable to the plan of care
