Job summary

Personal Touch Home Health Care Services Inc. is seeking a licensed Speech Language Pathologist to provide in-home and community-based therapy for older adults, promoting mobility, independence, and quality of life.

You will evaluate, plan, and deliver evidence-based therapy, document care in EMR, and collaborate with interdisciplinary teams, including caregivers. Telehealth options are used to increase access and flexibility.

Qualifications

Valid state licensure or eligibility in Kansas.
Degree from an accredited Speech-Language Pathology program.
Proficiency with EMR systems and basic computer skills.
Current CPR certification and ASHA CCC.
Ability to meet physical demands including lifting and driving between locations.

Responsibilities

Conduct evaluations and develop individualized treatment plans.
Deliver evidence-based therapy to older adults in-home and in community settings.
Document care accurately in electronic medical records.
Collaborate with interdisciplinary teams including PTs, OTs, and caregivers.
Educate patients, families, and staff to enhance safety and aging-in-place outcomes.
Utilize telehealth and virtual care options to increase access.
