Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to evaluate and provide speech-language pathology services in patients’ homes or facilities, under the supervision of the Clinical Manager. You will contribute to plans of treatment and care, and use professional judgment to assess and treat speech, voice, language, hearing, and swallowing disorders to improve patient function.

The role requires minimum six months of experience, an active license, CPR certification, and a valid

Qualifications

• Six months experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in the home or facility.
• Participate in development and review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to support rehabilitation.
