Job summary

You will collaborate with patients, families, caregivers, and an interdisciplinary team, delivering personalized care in a home setting while meeting licensure, CCC-SLP, and CPR requirements.

Qualifications

• Current, unencumbered license to practice speech pathology in the assigned state.
• Current CCC-SLP certification with accredited program coursework.
• Current CPR certification is required.

Responsibilities

• Provide in-home evaluation and treatment for speech, language, cognition, voice, and swallowing disorders in adults.
• Participate with patients, families, and interdisciplinary team to improve communication, safety, independence, and quality of life.
• Other duties as assigned by supervisor.
