Job summary

Senior LIFE Lehigh Valley is seeking a Social Worker for a full-time daylight position in Bethlehem, PA. The role supports home- and community-based services, completing assessments and coordinating care with interdisciplinary teams.

Responsibilities include case management for up to 50 members, counseling families, and documenting plans of care while meeting program and regulatory requirements. A Bachelor's degree in Social Services and a valid driver's license are required.

Qualifications

• Bachelor’s degree in Social Services is required.
• Must have a driver’s license, reliable transportation, and car insurance.

Responsibilities

• Oversee direct social work case management services for caseloads of up to 50 LIFE Center members.
• Provide social service support including screenings, counseling and external referrals as needed.
• Reevaluate and update Plans of Care based on changes in members’ status and communicate updates to stakeholders.
• Document all visits and maintain electronic case management assessments for immediate and future use.
• Identify development areas and implement improvements for LIFE Center programs and growth.
• Counsel families in individual and group settings and lead relevant educational activities.
• Visit in-home members, assess needs and develop/implement speech/language care plans as appropriate.
• Perform general administrative duties, including submitting required documents to agencies.
