Job summary

FOX Rehabilitation is growing in Bristol, TN, and we’re seeking licensed Speech Language Pathologists to deliver one-on-one care in homes or senior living communities. You’ll design plans of care and provide evidence-based treatment to improve mobility and independence.

Enjoy autonomy in scheduling, optional telehealth, and collaborative, interdisciplinary support that prioritizes patient outcomes and aging in place.

Qualifications

Valid Speech Language Pathology license in the state(s) of practice.
Degree from an accredited Speech Language Pathology program.
Basic computer and EMR proficiency.
Current CPR certification.
Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

Provide one-on-one Speech Language Pathology to older adults wherever they call home (private residences, senior living communities, etc.).
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, mobility, and independence.
Document effectively within FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety and aging in place.
Provide telehealth / virtual care when appropriate to extend access and flexibility.
