Job summary

FOX Rehabilitation in Omaha, NE seeks a licensed Speech Language Pathologist to deliver one-on-one care to older adults in private homes and senior living communities. You’ll develop personalized treatment plans, conduct evaluations, and provide evidence-based therapy within the Geriatric House Calls model.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Current Certificate of Clinical Competence (ASHA CCC).
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
