Job summary

FOX Rehabilitation in Wilmington, NC is seeking a licensed Speech Language Pathologist to deliver one-on-one care to older adults in private residences or senior living communities.

You will evaluate, create plans, and provide evidence-based therapy, with the flexibility to manage your own schedule and incorporate telehealth when appropriate.

Join a clinician-led, interdisciplinary team and contribute to meaningful outcomes while traveling to patient homes in the community.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice.
• Current Certificate of Clinical Competence (ASHA CCC).
• Proficiency with electronic medical records (EMR).
• CPR certification.

Responsibilities

• Provide one-on-one SLP services to older adults in home or senior living settings.
• Perform evaluations and develop individualized plans of care with evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document care within EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth when appropriate to extend access.
