Job summary

FOX Rehabilitation seeks a licensed Speech Language Pathologist to deliver one-on-one care in Fox Point, WI and nearby communities. You will assess and treat older adults in private homes and senior living settings using the Geriatric House Calls model.

This role offers autonomy, flexible scheduling, and the option to incorporate telehealth when appropriate, collaborating with a multidisciplinary team to promote safety, functional independence, and aging in place.

Qualifications

• Licensed Speech Language Pathologist in WI or eligible for licensure
• CCC required (ASHA)
• Experience with geriatric populations and home-based care preferred

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support wellness, longevity, mobility, and independence
• Document effectively in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
