FOX Rehabilitation is seeking licensed Speech Language Pathologists in Lenexa, KS to deliver one-on-one care to older adults in homes or Senior Living Communities as part of the Geriatric House Calls model.

You will evaluate, develop plans of care, and deliver evidence-based therapy, collaborating with an interdisciplinary team while maintaining flexible schedules including telehealth options.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsabilités

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
