Job summary

FOX Rehabilitation is expanding its Green Bay presence, seeking licensed Speech Language Pathologists to deliver one-on-one care to older adults in homes and senior living communities. You’ll conduct evaluations, craft individualized plans, and provide evidence-based therapy to improve function and independence.

Enjoy autonomy with a flexible schedule, telehealth options, and collaboration within an interdisciplinary team of therapists and clinicians, all while helping patients age in place.

Qualifications

• Valid SLP license in the state of practice or eligibility.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification and ASHA CCC credentials.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions focused on wellness, mobility, and independence.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
