Job summary

FOX Rehabilitation in Appleton, WI seeks a licensed Speech Language Pathologist to deliver one-on-one care to older adults in private residences or senior living communities, supporting mobility, independence and quality of life.

You will perform evaluations, develop plans of care, provide evidence-based therapy, document in FOX’s EMR, and collaborate with an interdisciplinary team to promote aging in place. Telehealth options and flexible scheduling enhance access.

Qualifications

Must hold a valid SLP license in the practicing state.
Degree from an accredited Speech Language Pathology program.
Basic computer and EMR proficiency required.
Current CPR certification is required.
Current CCC/ASHA credentials preferred.

Responsibilities

Provide one-on-one speech-language pathology services to older adults in homes or senior living settings.
Perform evaluations, develop individualized plans of care, and deliver evidence-based therapy.
Deliver therapy sessions that support wellness, mobility, and independence.
Document effectively in FOX’s EMR per regulatory standards.
Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and staff to promote safety and aging in place.
Provide telehealth/virtual care when appropriate to extend access.
