Fox Rehabilitation in Green Bay, WI is seeking a licensed Speech Language Pathologist to deliver one-on-one care in homes and senior living communities.

You will evaluate patients, develop individualized plans of care, deliver evidence-based therapy, and document in FOX’s EMR system, collaborating with PTs, OTs, SLPs, and Fitness Specialists. This role offers autonomy, flexible scheduling, optional telehealth, and meaningful outcomes that help older adults age in place.

Qualifikationen

• Licensed Speech-Language Pathologist in the practicing state(s).
• CCC-ASHA certification or eligible to obtain.
• Degree from an accredited Speech-Language Pathology program.

Aufgaben

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
