Job summary

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Basic computer proficiency and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one speech language pathology services to older adults in their homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support patient wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system in compliance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/ virtual care when appropriate to extend access
