Job summary

FOX Rehabilitation seeks a licensed Speech Language Pathologist to deliver in-home and senior living care. You will perform evaluations, craft care plans, and provide evidence-based therapy tailored to older adults in private residences and communities.

Flexibility to create your own schedule, potential telehealth components, and collaboration with a multidisciplinary team are offered to support patient outcomes and clinician growth.

Qualifications

• Valid Speech Language Pathology license in the state of practice, or eligibility to obtain.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote aging in place.
