Phoenix Home Care & Hospice is seeking an In-Home Registered Nurse or Licensed Practical Nurse to join our compassionate team in Springfield, MO and surrounding areas. You will conduct in-home visits, perform assessments, manage medications, and support patient families with education and care planning.

We offer a $2,500 sign-on bonus, flexible full-time or part-time schedules, and comprehensive health plans along with PTO, a 401K, and supportive in-home team culture.

Formación

• Active Registered Nurse or Licensed Practical Nurse license in Missouri.
• Home care experience preferred but not required.
• Strong organizational and self-management skills.
• Excellent customer service and compassionate bedside manner.
• Valid driver’s license.

Responsabilidades

• Complete admission assessments and reassessments for in-home clients.
• Provide nurse visits for adults and seniors: vitals, medication management, diabetic nails, and labs.
• May perform supervisory visits for caregiver services.
• Educate patients and families on medications and care plans.
• Maintain accurate documentation and time management.

Responsibilities:

• Complete admission assessments and reassessments for In-Home caregiver population.
• Provide nurse visits for adult and senior population: Taking vitals, medication management, monthly diabetic nails, and labs.
• May provide supervisory visits for caregiver services
• Provide instruction/ education on medication
• Good time management, detail oriented and self-starter

Requirements:

• Active Registered Nurse or Licensed Practical Nurse license in the state of MO
• Home Care experience preferred but not required
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver’s license
