Job summary

All About Caring Home Care LLC in the Minneapolis–St. Paul metro area is seeking an experienced Per Diem Registered Nurse (RN) to conduct client assessments, develop ISPs, and provide clinical oversight in private-pay home care.

This flexible, field-based role offers autonomy and work–life balance for an RN looking to supplement hospital, hospice, or clinic work. The position requires a Minnesota RN license and at least 3 years of nursing experience, with strong clinical judgment and knowledge

Qualifications

• Active RN license in Minnesota
• Minimum 3 years nursing experience (home care, hospice, or community health preferred)
• Knowledge of MN home care licensing rules and client assessment procedures
• Excellent assessment, clinical documentation, and communication skills
• Reliable transportation and valid driver’s license
• Self-motivated and organized with strong time management skills

Responsibilities

• Conduct initial, comprehensive nursing assessments and periodic reassessments in clients’ homes.
• Develop, implement, and update individualized service plans (ISPs) per MN home care rules.
• Provide clinical oversight for Caregiver, CNA, and HHA staff; perform supervisory visits as required.
• Ensure all documentation meets state licensing and agency quality standards.
• Communicate effectively with clients, families, caregivers, and interdisciplinary team members.
• Participate in quality assurance, compliance, and clinical training initiatives.
• Maintain professional ethics, confidentiality, and high standards of nursing practice.
