Qualifications

• Active RN or LPN license in the state where the position is based (Massachusetts).
• Prefer 2+ years of nursing experience with a geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and strong communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial patient and family assessments to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create Plans of Care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and integrate them into care plans.
