Job summary

Pathways Healthcare, a leading home health and hospice organization in Massachusetts, seeks skilled nurses to deliver patient-centered care in the home. The role focuses on geriatrics, with comprehensive assessments, medication administration, and collaboration with physicians to build tailored plans of care.

Applicants must hold an active MA RN or LPN license, with 2+ years of nursing experience preferred.

Qualifications

• Active RN or LPN license in MA; LPNs encouraged.
• Preferably 2+ years nursing experience in geriatrics.
• Current BLS CPR certification.
• Possess a valid driver's license and reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial patient and family assessments to determine home care needs.
• Develop and revise individualized care plans based on patient status.
• Collaborate with physicians, nurse practitioners, and teams to create plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them accordingly.
