Job summary

Select Home Health Services, Inc. in Carmel, IN seeks a skilled Registered Nurse to provide in-home nursing care, perform patient assessments, and implement physician-ordered plans of care.

The role involves coordinating services, educating patients and caregivers, and supervising LPNs/HHAs as needed to ensure high-quality, personalized care at home. Qualifications

• Graduate of an accredited nursing program and licensed as an RN in Indiana.
• Minimum 1 year of nursing experience in hospital, facility, or home health.
• Valid driver's license, reliable auto insurance, and personal transportation.
• Current TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications and disease processes.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.
